Primarily day shifts with occasional flexibility ATTRIBUTES BSN or ASN with a minimum of 2 years Home Care experience.
Clinical expertise through experience.
Certifications in clinical topics and/or leadership preferred.
Computer proficient.
Travel required.
RESPONSIBILITIES Leadership.
Provides leadership and direct supervision of assigned clinical staff to include but not limited to:
co-visits with staff, OASIS supervisory visits and competency/clinical supervisory visits per policy, documentation oversight, case management and case conferencing.
Case Management.
Case management of patients, documentation in NetSmart required.
Onboarding.
Participates in the professional staff orientation process as assigned.
Provides professional staff with education/mentoring in regards to work performance, performance improvement, in-service and skill development.
Participates in interview and on-boarding processes for new staff.
EDUCATION:

Nursing (Required)LICENSES / CERTIFICATIONS:
BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE:
$70,000.
00 - $90,000.
00 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
It may be modified in the future and eligible for additional pay components.
Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.

The check will bounce and the victim is left responsible.
